You need to replace the text highlighted in red with the image link.

So the end result will look something like this:

And like Michael said, it goes in the Global Header.

From what I have read at other forums that he has this template on, there is no other download link at this time. As far as I know, Stinky was informed over a month ago about this.

Unless someone has permission to rehost Stinky's template for download, or if he rehosts it himself, there isn't anything anyone can do. You can always go over to his forum and ask Stinky himself, Extreme Data.

Hello, I must admit, I love this template, but the only thing that is stopping me from using it, and yes, I am picky. But, it is the... Folder Icons. I just wish it could be different, somehow. I know it may sound weird, and I don't know really how to describe it... But, keep up the good work!
