I'm having a hard time trying to set this template. Well, mostly trying to find where to put the URL's of each icon and everything in the header and footers.

For the icons, such as the folders, rank images, menu buttons, etc, you must go to the Customizable Images which you can get to through the Forum Colors / Skins admin power. When you go there, there is a text link to the far right that says "Customizable images."
The rest of the images you may need, for instance the Cat, Title, Nav, Menu images and the bg image... And the head/base images. The code will say, "URL HERE," or something to that effect which will guide you in where to put the URLs.

could someone just give me a full code to set this up? i cannot download t because my computer is wayyyyy too slow but i really love this design!

The only way you can set this up is to Download. All the codes needed are provded in the Zip folder that you will receive after download.
